By submitting this completed form, I understand that the above information will only be used by Woodford Cricket Club to promote their club, events and activities and will not be passed on to any other people or organisation outside the Club without my express permission.

Parents/Guardians of the Woodford Junior Cricket section, I agree to my son/daughter/child in my care to take part in the activities of the club and that a parent/guardian should be present whilst Woodford Junior Cricket is taking part in the activities. I understand that I will be kept informed of these activities – for example, timing and transport details and that in the event of any injury or illness, all reasonable steps will be taken to contact me, and to deal with that injury/illness appropriately.

I understand that any team or activity photographs/videos taken will only be used for coaching purposes and for club promotion such as its brochures,  website or social media.
